PRIMARY MEDICAL CARE

Same day acute care appointments for colds, coughs, flu, bronchitis, hepatitis treatment, ear aches, burns, cuts, bruises, sprains, rashes, skin conditions, and other illnesses that have occurred in the past 24-48 hours is available. Individuals receive direct healthcare and coordination of specialty medical care for chronic diseases and illnesses including asthma, diabetes, hypertension, heart problems, muscular and skeletal problems, high cholesterol, nicotine habit, cancer care, and stomach problems.

INDIVIDUAL, FAMILY, & GROUP THERAPY

Help, including individual, group and family therapy, for those with emotional problems is available.

INTENSIVE IN-HOME SERVICES

Intensive In-Home (IIH) teams are available to serve children and adolescents who are at risk of out-of-home placement and/or need to be stabilized within the home.

THERAPEUTIC FOSTER CARE

For children who need a safe and nurturing environment, Therapeutic Foster Care (TFC) can help. The goal for TFC is for each child to return to his or her natural home having made positive behavioral changes.

SUBSTANCE USE SERVICES

Comprehensive Substance Use (SU) Services are offered, including both individual and group therapy; Intensive Out-Patient Treatment; Substance Abuse Comprehensive Out-Patient Treatment; Dual-Diagnosed Treatment; Gender Specific Treatment; DWI Treatment Services and Peer Support Services. Within the SU Comprehensive Services, a team of Behavioral Health Care and Primary Care professionals work together with individuals (and their guardians, if appropriate) to develop a plan to address goals centered around attaining and sustaining recovery in order to achieve healthier lifestyles.

PSYCHIATRIC CARE & MEDICATION MANAGEMENT

Psychiatric care and medication management assessments are available for individuals dealing with mental health challenges such as bipolar disorder, schizophrenia, depression, and anxiety.

ASSERTIVE COMMUNITY TREATMENT TEAM (ADULTS ONLY)

The Assertive Community Treatment Team (ACTT) provides a comprehensive personalized treatment program for individuals with a severe and persistent mental illness. ACTT staff work with people in their home, at school, or at work and are available 24/7. The ACTT employs a team approach to help individuals with everyday basics such as taking medications and making it through the day. The team includes a psychiatrist, nurses, vocational specialists, therapists, other mental health professionals, and peer support staff to help people remain out of the hospital and living in the community.

SUPPORTED EMPLOYMENT

Supported Employment provides adults who have significant disabilities and challenges with planning, development, job coaching and supports.

SUBOXONE TREATMENT

Help is available to those addicted to heroin, methadone, or prescription pain killers. With the use of the drug Suboxone, this service provides individuals with the ability to detox without pain or expensive inpatient hospitalization. Suboxone reduces the cravings for opioid drugs while individuals learn to live a drug-free life by participating in treatment and slowly decreasing the dosage of Suboxone.

EMPLOYMENT & TRAINING SERVICES

Core services are provided through the local NC Works Career Center including: job search and placement assistance (including career counseling); labor market information (which identifies job vacancies, skills needed for in-demand jobs, and local, regional, and national employment trends); initial assessment of skills and needs; information about services; and follow-up services to help individuals keep their jobs once they are placed. This program serves both drop out youth and the adult populations.

PEER SUPPORT

Certified Peer Support Specialists are individuals with lived experience who now, through their own recovery, asisst others with mental health and or substance abuse disorders. Support is provided to individuals related topersonal needs (housing, food, healthcare and employment), connections with crisis.

ACCESS TO MENTAL HEALTH, SUBSTANCE USE, & INTELLECTUAL/DEVELOPMENTAL DISABILITIES SERVICES

The 24-hour Customer Services Crisis Line offers offers immediate help to people in emotional crisis. It is a unique resource for family and friends of persons with mental illness or those considering suicide. Call TOLL FREE, 1-888-235-HOPE (4673).

RN CARE COORDINATION & CARE MANAGEMENT

Community Care of NC/AccessCare Care Managers and Care Coordinators, employed by Partners Behavioral Health Management, work with individuals and his or her natural supports to make sure that people are cared for in the best possible manner. A nurse care manager/care coordinator are available to act as an advocate between medical and behavioral health providers, to assist with understanding medications, to make sure all community resources are shared, and provide disease education.

Burke Integrated Health is equipped to make necessary referals to services located at other sites with a variety of providers and professionals.
